Admission packets with all the required forms can be found in the front office.

Registration Information

New students must submit a new student enrollment form, the tuition contract, and a deposit.

All students must provide the following documents prior to the start of the school year:

• Birth Certificate
• Proof of Immunizations
• Proof of Vision Screening
• Diocesan Student Registration
• Baptismal Certificate
• Consent for Disclosure of Immunization Information
• Covenant
• Request for Education Records (if applicable)
